Our IVF refund plan provides a stated percent refund of in-cycle expenses if a
pregnancy sustained through the first trimester (12 weeks) does not result. Although
multiple transfer cycles may be required, all frozen embryos resulting from the initial
egg retrieval must be transferred in order to qualify for refund. The refundable expenses
for clinical and laboratory services associated with the IVF cycle are outlined below. The
program applies only to those couples and individuals for whom IVF is medically indicated.
This plan is applicable to ovum donation, but not situations requiring a gestational
carrier.In-cycle services provided under plan:
- medical services provided by a PFC physician in our facilities, including clinical
monitoring, egg retrieval, embryo transfer and in-cycle medical consultations related to
IVF
- basic services of the IVF laboratory, including the in-vitro fertilization procedure, and
cryopreservation and storage of embryos for one year.
- subsequent transfer of any frozen / thawed embryos within a one-year period.
Costs not covered and not refundable under plan:
- pre-cycle evaluation and lab testing
- medications for ovarian stimulation and preparation of the uterus for optimal
implantation
- anesthesia during the egg retrieval procedure
- pregnancy tests: obstetrical ultrasound
- facilities charge
Special services not required by all patients-costs not covered and not refundable:
- services of a non-PFC physician, such a as a urologist performing a Testicular Sperm
Extraction (TESE) procedure or an obstetrician monitoring a patient outside of a PFC
facility
- any other medical treatment or monitoring provided by a non-PFC physician and / or
facility
- Intracytoplasmic Sperm Injection (ICSI) fees or other specialized laboratory services
- donor sperm
- agency and ovum donor fees
IVF Refund Plan Fees
Fees for the IVF Refund Plan are based upon the age of the woman providing the eggs at
the commencement of the cycle. The date of commencement of the cycle is determined by the
start date of the fertility medications used to stimulate the production of multiple
follicles.
Medical Criteria:
- IVF/ET must be medically indicated.
- The female partner must use her own eggs, or those of an acceptable donor and her own
uterus.
- The female partner must meet medical criteria based upon review of past medical history, physical examination, precycle lab work including measurement of FSH and estradiol, and documentation of a normal uterine cavity, free of pathology. If using her own eggs, she must be less than 39 years of age.
- Medical and psychological evaluation must show that the woman is physically and mentally capable of carrying a pregnancy and giving birth.
- An appropriate sperm source must be provided by the individual or couple.
- The couple or individual must comply with all medical protocols and directives related
to treatment prescribed by the PFC physician.
- The number of fresh or thawed embryos transferred per cycle will be decided in
consultation with a PFC physician, but the couple or individual may choose to transfer as
few as two (2) embryos per cycle of treatment
- All embryos resulting from the fertilization of eggs removed during a single egg
retrieval procedure must be transferred either fresh or frozen/thawed even if this takes
several cycles to accomplish, unless the PFC physician determines that additional embryo
transfer procedures are not medically advisable.
- The couple or individual must sign all informed consent and financial agreement
documents.

NOTE: Must meet inclusion criteria to participate in this plan.
All fees are subject to change without advance notice. If the couple or
individual and PFC have executed a financial agreement, the price stated in the agreement
will be honored for the cycle, regardless of any change in fees occurring after the
